A choice of HMO, PPO or HSA-PPO plans for employees and dependants with Dudek sharing the cost of premium payments

Life and Accidental Death and Dismemberment Insurance
Life and Accidental Death and Dismemberment (AD&D) benefits for all full-time employees. Life benefit is one-times salary to a maximum of $50,000

Long-Term Disability Insurance
Long-term disability benefits for all full time employees

Voluntary Life Insurance
Additional individual life insurance coverage may be purchased by full time employees. Covered employees may add life insurance for their dependents.  Individual life insurance is offered at low group rates and is portable.

401(K)
Available to all regular full-time employees. Dudek annually matches employee contributions at a discretionary rate.

ESOP
Employee Stock Ownership Plan (ESOP). All full-time employees participate.

Section 125 Cafeteria Plan
Employees may use pretax dollars to pay for certain un-reimbursed medical expenses and dependent day care expenses.

HRA - Health Reimbursement Arrangement
Dudek contributes a discretionary amount annually toward non-reimbursed medical expenses for all full time employees.
